Fortunately, the editor did not let the concept die of inanition, but instead added eight contributors and six chapters, thus producing a volume that borders on being a textbook.Adding a chapter on complications of vascular access allowed Eric Wilson to expand upon some ideas expressed in his textbook on this subject of vascular access. John Porter's chapter, the best in the first edition, has been totally rewritten by the author and his colleagues at the Clinical Research Center of the University of Oregon's School of Medicine, Portland. Here, the diagnosis and treatment of vasospastic disease are upgraded and updated and the reference list has been more than doubled.
