The efficacy of this treatment is now well documented. The difficulty is, as noted by Eason and colleagues, when the patient has obstruction of the collecting system. Retrograde catheterization has been used in such patients, and those references were quoted by the authors. An alternative approach is available for the obstructed patient with uric acid lithiasis. This involves the use of percutaneous nephrostomy for urinary diversion associated with the conservative medical treatment already well described. We have published an article describing this technique and the results in two patients,1 which appeared after the article by Eason and co-authors.
