The authors have considered every question from the standpoints of pediatrics and of surgery. In their experience they found that certain questions recurred frequently and they have attempted to present the answers to these. Questions relating to malformations were frequent, and this subject has been covered at length. While there was a certain division of authorship in the various portions of the different chapters, each was jointly edited so as to avoid any conflict of advice. Only in a few instances has the point of view of the authors been irreconcilable, and in these cases each has presented his point of view and has allowed the choice of method to the reader.
