The basic structure comprises alphabetized diagnoses, each a maximum of 3 pages and each following a structured format that opens with key clinical points and proceeds through basics (description, epidemiology, incidence, risks, pathophysiology), diagnosis (signs and symptoms, history, physical examination, testing), treatment, and follow-up. Although the preface describes the text as evidence-based, it does not attempt to report levels of evidence for the numerous recommendations for evaluation and treatment or to cite genuinely evidence-based references. Indeed, a significant number of topics cite references such as American College of Obstetricians and Gynecologists bulletins, textbook chapters, and Up to Date online. While these are perfectly legitimate resources, they do not qualify as evidence-based, and most experienced clinicians will quickly recognize the recommendations as an amalgam of expert opinion, consensus guidelines, and “standard of care.”
