To the Editor: The claim made by Dr Colla and colleagues1 of savings under the Medicare Physician Group Practice Demonstration (PGPD) apparently excluded some Medicare payments to PGPD (but not control) sites (ie, bonuses for hitting quality and use targets). In other words, partial Medicare payments to PGPD sites were compared with total payments to control hospitals. The Congressional Budget Office, which also analyzed the PGPD, concluded that it had “little or no net effect on Medicare expenditures, after accounting for the bonuses. . . . ”2
