Sites and topics newly covered in this edition include the extrahepatic bile ducts, distal bile duct, cutaneous squamous cell carcinoma, Merkel cell carcinoma, and adrenal gland. All sites and topics covered in previous editions, including the bibliographic references, have been updated. The section on ophthalmologic malignancies has been greatly expanded. The updated TNM definitions go into more detail, particularly for lymph node status in breast, lung, and colon sites. TNM criteria for each tumor and histologic type are defined, and nonanatomical prognostic factors are applied. These relevant markers include ER, PR, ERBB2 (formerly HER2 or HER2/neu for breast, KRAS for colon and rectal cancers, as well as Gleason score for prostate cancer and mitotic rate for gastrointestinal stromal tumors. Different anatomical sites show large differences in tumor treatment and outcome. This edition recognizes that melanoma of the skin, uvea, conjunctiva, and head and neck mucosa behave differently and are staged accordingly.
