Surgical complexity and risk, along with the burden of acute and chronic disease, combine to define a patient's perioperative risk. Despite the increasing number of extremely sick operative candidates, advances in anesthetic techniques and monitoring have led to an expectation of operative survival in all but the most extreme situations. Thus, a key challenge to the profession of anesthesiology must shift from simply getting the patient out of the operating room alive to maximizing longer-term goals of survival and optimal function.
