In Reply: Dr Eckert and colleagues are correct that there is an error in the Box in our article. The 3 variables they cite are indeed missing. We reviewed the study by Shapiro et al1 and agree the 3 variables should be included in the rule. However, in doing so, we noted that the Shapiro et al report also seems to have an error in that their Table 3 lists the rule but omits the variable neutrophils greater than 80%. As a result, we contacted Dr Bates, one of the authors of the original article, to determine whether this omission was also unintentional. He confirmed that it was.
