To the Centers for Medicare & Medicaid Services (CMS), a readmission is an admission to a hospital that occurs within 30 days of a discharge from the same or another hospital. Excess readmissions are measured as a ratio of a hospital's readmission performance compared with the national average for the set of patients with an applicable condition, and it is based on 3 previous years of discharge data and a minimum of 25 cases of patients with the specified conditions.2
