P4P is spreading as public and private payers address what many experts describe as the inexorable and unsustainable escalation of spending for health services. The provisions of the Affordable Care Act that address cost containment are expanding the number of institutions and medical practices that participate in P4P programs. The authors conclude that “some P4P projects have achieved sufficient efficiencies to cover operational costs and still net additional savings to the Medicare program” (p 328). The authors also demonstrate their command of the literature on P4P, including many highly technical issues, in straightforward prose that takes account of the enormous variation in the expertise of potential readers.
