On the first day of my internship on July 1, 1992, as a transitional-year resident at a Philadelphia community hospital, I thought I spoke good English, and because I was a board-certified surgeon from India, I thought I was knowledgeable about medicine. I quickly realized in taking the history and examining my initial patients that I did not know much about American culture and medicine. My transitional year of residency was literally a history-and-physical machine for me, as I did H&Ps on 15 to 20 patients each day, trying to learn more and more about American medicine and Americans in general.
