First, the pathophysiology of diabetes in lean or normal-weight adults is heterogeneous, including impaired insulin secretion, increased endogenous glucose production, high inflammatory state, and higher stress hormones; any of these characteristics may contribute to higher mortality. However, incomplete control for confounding or reverse causation may still remain in the study by Carnethon et al,1 particularly because the authors did not present data about renal function at baseline.
