Therefore, the numerator is an estimate of the total number of AAS users in population P divided by the total population P. With that comment aside, the issue of using rate as a generic term for both rates and proportions has not been settled, almost 20 years after the Elandt-Johnson article.1 An epidemiology instructional guide from the Centers for Disease Control and Prevention2 and textbooks such as Lilienfeld and Lilienfeld3 and Mausner and Kramer4 define "prevalence rate" of a given disease as the proportion of existing cases at a specified time in the population at that specified time.
