To the Editor.— I agree with Dr Schmidt's1 comments on the technique for vasectomy in that ligation of the vas is far from being any guarantee of its permanent obliteration. I differ, however, in only fulgurating the urethral end of the vas. I then suture the sheath of the vas over the testicular end.Fulguration of the testicular end will lead to the likelihood of blowouts in the epididymis and this will decrease the success of later vasovasostomy. I consequently do not have a problem with there being a sperm granuloma near the site of the ligation, as long as the urethral end is satisfactorily fulgurated.
