An elderly woman contracted transfusion-induced babesiosis, accompanied by disseminated intravascular coagulopathy. Her parasitemia cleared after treatment with several antibiotic and antimalarial drugs, but she died of cardiopulmonary failure. One of the transfusion donors had a high indirect fluorescent antibody titer against Babesia microti but was asymptomatic; his parasitemia was detected only by hamster inoculation, illustrating the difficulty in detecting the carrier state.
