A case is reported of arthritis associated with recurrent alcoholic pancreatitis. Unusual features were the long-term course of 2 1/2 years, persisting during a nine-month remission of the pancreatitis; lack of response to anti-inflammatory therapeutic agents; and the sequel of residual joint damage. Sequential observations on the enzyme and lipid patterns of the patient's serum and synovial fluid during the course of the chronic arthritis provided no direct evidence for increased pancreatic lipase activity. Low complement and high prostaglandin E levels, found in the synovial fluid, associated with normal serum levels of both, indicated the involvement of these mediators in the persisting synovitis.
