To the Editor.—  Those who practice medicine in teaching hospitals have come to expect the period from late June to mid-July to be tumultuous. It is the time when residents depart for further training or for jobs and when new residents arrive either fresh from medical school or for secondary training in specialty or fellowship programs. With the proliferation of such secondary programs, the transition period has become unpleasant and disorganized. Residents seeking secondary training frequently find the necessity to leave their old programs early, which dumps their clinical work load and on-call responsibility on their remaining harried colleagues. The resident cohort available to assist in the indoctrination of new residents and to smooth the transition of clinical services is thus reduced.July 1 is often the first practical contact new residents have with their new hospital. Some institutions have introduced orientation periods of 1 or 2 weeks to assist
