The editors have graciously allowed me to reply to those who have responded to my recent note.
My personal mail has been flattering; not so The Journal's. Of interest is the fact that 80% of my mail (300 letters in six weeks) has come from university-based physicians. These are undoubtedly the more long-suffering of those who have been "advised" of their "position" on the team.
In the penultimate paragraph of the criticism accompanying this reply, Dr Etzwiler states that "if we [physicians] choose not to accept..., there are others who can, are willing, and perhaps should assume such a position."
Without any false humility whatsoever, I reply, "Who are they?"
