Bilateral Testicular Tumors in Untreated Congenital Adrenocortical Hyperplasia

Congenital adrenal hyperplasia in a 40-year-old man was undiagnosed until bilateral interstitial cell tumors of the testicles developed. Adrenal insufficiency developed following excision of the testicular tumors. Studies led to diagnosis of congenital adrenal hyperplasia. Surgical exploration for a hemorrhagic adrenal cyst allowed comparison of adrenal and testicular tissue. Testicular tumor histological findings were not similar to those of the adrenal gland. The association of testicular interstitial cell tumors with chronic excess adrenocorticotropic hormone (ACTH) stimulation is rare but has been reported.
