Wisconsin; South=Alabama, Arkansas, Delaware, District of
Columbia, Florida, Georgia, Kentucky, Louisiana, Maryland, Mississippi,
North Carolina, Oklahoma, South Carolina, Tennessee, Texas, Virginia,
and West Virginia; and West=Alaska, Arizona, California,
Colorado, Hawaii, Idaho, Montana, Nevada, New Mexico, Oregon, Utah,
Washington, and Wyoming. Models including state-specific controls
yielded results similar to those obtained with controls for region of
the country. Because sample sizes in subpopulation analyses were
smaller, region of the country rather than state-specific controls were
used in all models.
