This multiauthored text skillfully addresses the occurrence, diagnosis, and management of acute kidney injury and renal failure. Some chapters overlap substantially with others, yet others make up for that by their quality and importance. The chapters on hepatorenal syndrome, hemodynamic monitoring, and stopping dialysis were exceptionally useful and pragmatic. The reader is reminded of numerous medically important and applicable facts—eg, that urinary sodium level is not a criterion for the hepatorenal syndrome; that use and benefit of dynamic monitoring in the intensive care unit are increasing; that a delayed nephrology consultation may adversely affect survival; and that little evidence and few criteria are available for determining when renal recovery is sufficient to stop dialysis.
